Oblique Lateral Lumbar Interbody Fusion Procedure

The following workflow presents a general OLIF surgical sequence. Final approach, implant selection and posterior fixation strategy should be determined by the surgeon according to the individual case.

Patient Positioning and Surgical Access

The patient is positioned in the lateral decubitus position. A minimally invasive oblique lateral retroperitoneal approach is performed to access the target disc level.

Disc Removal and Endplate Preparation

The affected disc is removed, and the cartilaginous endplates are carefully prepared to create an appropriate fusion bed while maintaining endplate integrity.

OLIF Cage Insertion and Restoration

An OLIF cage filled with the selected graft material is inserted into the disc space to restore disc height, support sagittal alignment and provide anterior column support.

Posterior Fixation

An open or minimally invasive pedicle screw-rod system is applied according to the surgical plan to provide supplemental stability and maintain spinal alignment.

Final Verification

Fluoroscopic imaging is performed to confirm cage position, implant stability and spinal alignment before final wound closure.
